New Patient Scheduling & Policies

Insurance Accepted:  Blue Cross Blue Shield, United Healthcare, Cigna, ComPsych, Humana , Magellan, Sana, & some Aetna plans.

Self-Pay Rates for an Initial Evaluation is $300.

Please Note: We do not complete disability paperwork for new patients that have not been previously established under our care.

New Patient Office Policies:

  • All new patient forms must be completed by 5:00 pm the day before your appointment. If the forms are not completed in a timely manner your appointment will be cancelled and rescheduled.

  • Payment is due at the time of service.

  • For telemedicine appointments, please be sure you are logged into your MYIO app a few minutes before the time of your appointment.

If forms are not complete, payments is not collected, and/or our providers are unable to connect with you then your appointment will be canceled and may be subject to cancellation fees.

Established Client Policies

Please keep in mind…

  • Refill requests MUST be submitted directly through our office using the MYIO Patient Portal. Please do not submit refill requests through your pharmacy.
  • You can now self-schedule in the MYIO App.
  • We do not do same-day refill requests, please allow 3-5 business days for processing of refill requests between appointments.
  • Our office does not refill medications after hours.
  • You may be required to see your provider before your medication is refilled if you have missed your prior appointment(s).  Patients on controlled medications are required to see their provider every 90 days.

*It is important to note that ADHD medications are not considered vital medication and running out does not constitute a medical emergency.  Early refills are not permitted under any circumstances. In the event that medication is stolen, patients must provide proof of a filed police report.
